Automobile Accident

In the event that you are involved in an automobile accident while driving on University business, please complete the following form and return it to Risk Management Services.

• Automobile Accident Report

Additionally, an Accident Incident Report form is to be completed and sent to Environmental Health and Safety. 

Property Damage

If you have sustained property damage please complete the following form and return to Risk Management Services.
